How to Create a Membership Form in ExplaraX?
- From the left Main menu, click on Form, then select Add Form. This will open the Create Form panel.
- Enter the Form Name and Description.
You can:
- Import system-provided questions,
- Reuse questions you have created earlier, or
- Add custom fields (these can also be added later after the form is created).
Note: “First Name” and “Last Name” fields are included by default in every Membership Form. You do not need to add them again.
